Type 1 Diabetes

How to Register for the NDSS: Step by Step

Step 1: Get a Formal Diagnosis

You need a confirmed diabetes diagnosis from your GP or specialist before you can register. If you have recently been diagnosed and are not sure what to do next, our pharmacists are a good first point of contact.

Step 2: Pick Up Your Registration Forms

Visit us at 98 Marine Parade, Southport. We stock the correct NDSS registration forms for Type 1, Type 2 and gestational diabetes. Just come in and ask.

Step 3: Have Your Doctor Sign Off

Take the form to your next GP appointment. Your doctor completes the medical section and signs off on your registration. Most GPs are very familiar with the process.

Step 4: Return the Form to Us

Bring the completed form back to Greg Keily Chemist and we will process your NDSS membership in-store. You can start accessing subsidised products on the same visit.
